The film, inspired by a woman who suffered from hypopituitarism, will show more about Esther's dark past. The adult killer who looks like a child will try to escape from a psychiatric clinic in Estonia and travel to the United States to torment a family in Connecticut.
Isabelle Fuhrman, twenty-five years old, returns for this prequel in the role of the villain playing a character who appears to be nine years old thanks to makeup, camera work, and body doubles. The cast also includes Julia Stiles (Hustlers) and Hiro Kanagawa.
